Absentee Request Information

To complete the Absentee Ballot Request Form you can print a blank form and fill it out and return it to our office or you can complete the fillable parts of the form, then print the form. The voter's original signature is required on the form prior to return it to our office. Applicants must enter the name field, select the appropriate election, enter the voter's residential address, city, state and zip. If you are requesting the ballot be mailed to you at an address different that your residential address you must also complete the mailing address fields. Voter's must also include the last 4 digits of their SSN, their DOB and a contact phone number. All applications must mark a reason they are applying to vote absentee and the form must be signed by the applicant. There is a field for a voter with a disability that is unable to sign their own name to make a mark.  
 
Absentee ballots can be returned by mail or in person, they must be notarized unless you are voting absentee due to incapacity or confinement due to illness, are a caretaker or are a military voter. 

 

PLEASE NOTE: Ballots returned by mail, the USPS strongly suggests placing your Absentee in the mail for as soon as possible in order to ensure it is received at the Election Office by 7 p.m. on Election Day. Ballots returned in person must be returned by the voter themself or by a relative of the 2nd degree.
